Eligibility Criteria
Most UofA students, UofA employees, and some affiliates are eligible for services at Campus Health. While health insurance is never required, Campus Health accepts most major health insurance plans and also offers options to help keep costs low.
Students
- UA students who are currently enrolled
- Most continuing but not currently enrolled UA students
- Most UA students on sanctioned leave of absence
- Most new incoming UA students
Note: Arizona Online students will be charged a $10 daily user fee.
If you will be under 18 when classes begin, your parent/guardian will need to complete the the Consent to Treat Minor form and submit that form here prior to using services at Campus Health.
Employees
- All current University of Arizona employees
- Some University of Arizona employees who have sustained job-related injury or illness
University Health Insurance Plans are accepted and most other United Healthcare, Cigna, Aetna, AND Blue Cross Blue Shield plans are also accepted.
Affiliates
- Exchange Visitors (J-1)
- Affiliate groups with service agreements with the Campus Health Service
