Coverage Period Options

*Can enroll prior to but no later than 30 days of appointment date or qualifying event.
If you enroll and do not maintain your coverage through the end date of your appointment, you will not be allowed to re-enroll onto the plan unless you have a new appointment.

Required Documentation to Enroll

  • Host department must arrange for an EMPLID
  • Once EMPLID is obtained, submit Enrollment Application
  • Notice of Appointment

Renewal Notices are mailed to the address noted on your enrollment application, if enrolled to the end of a coverage period.

Quick Process to be Used in Obtaining an EMPLID:

This is specific to a DCC enrolling onto the Student Health Insurance.

An EMPLID can be requested as soon as the visitor has been invited to the University.

Because the DCC is still out of the country, you can respond to the DCC transaction as follows: 

  • 1st question: Is the person a US Citizen or Person Resident = Select NO
  • 2nd question will appear when NO is selected: Will the person be collaborating or performing their services in the US = Select NO

This will bypass the Visa Document requirements, and will allow the department to proceed with the DCC process and get an EMPLID. 

Additional Information for Departments: Add comment to explain DCC is currently out of the country, and visa documents should be hand delivered to Systems Control when DCC arrives to the country.

All Exchange Visitors (both J-1 principals and J-2 dependents) are required to have sickness, accident, medical evacuation, and repatriation insurance in effect for the entire duration of their EV status. A willful failure to carry insurance is considered to be a violation of the EVP regulations. 

US Department of State Requirements

62.14 - Insurance

  • medical benefits of at least $100,000 per accident or illness
  • a co-payment not greater than 25% of the covered benefits per accident or illness
  • a deductible not to exceed $500 per accident or illness
  • a waiting period for pre-existing conditions that is reasonable by current industry standards
  • coverage for activities inherent to the exchange program (i.e., flight training for an aviation school)
  • repatriation of remains in an amount of $25,000
  • expenses associated with the medical evacuation of the exchange visitor to his or her home country in the amount of $50,000

The US Department of State (DOS) requires all J1 Student Interns to have insurance.  J1 Student Interns are eligible to enroll for The University of Arizona Student Health Insurance Plan with coverage taking effect the date they arrive to the United States through the date of departure from the United States plus two days to allow for any circumstances that may hold up the actual departure. You may not change coverage during this period. Coverage dates are based on travel itinerary. J1 Student Intern may be charged an additional 30 days of coverage, with no refund, if departure documentation is not provided at time of check-in.

Purchasing the Student Health Insurance

  • Host department must arrange for an EMPLID
  • Once EMPLID is obtained, submit Enrollment Application
  • Once Enrollment Application is received, an invoice for 30 Days of Coverage will be emailed to department and Student Intern. Wire transfer information will be included in the invoice.

Once initial payment is received, a Proof of Insurance for 30 days will be sent to International Student Services (ISS) and the Student Intern.

Travel Itinerary

  • After DS-2019 is received and a visa is issued, make your travel arrangements and provide the Campus Health Insurance Office your travel itinerary showing date of entry and departure from the United States. Submit travel itinerary securely to:  https://web.health.arizona.edu/cgi-bin/secure/insform.php
    • When this information is received, the Campus Health Insurance Office will issue an invoice to the student intern for the remaining premium amount due.
  • Upon arrival to The University of Arizona:
    • Check-in with the Campus Health Insurance Office
    • Present travel itinerary (if not already provided)
    • Present DS-2019
    • Pay remaining premium (if not already paid) by money order or cashier’s check
  • UA Campus Health Insurance Office will:
    • Issue a Proof of Insurance document
    • Provide an overview of the insurance benefits
    • Email a Final Proof of Insurance to ISS and Student Intern
