Arizona Board of Regents Student Health Plan
administered by The Chickering Group - an Aetna Company
Eligibility
requirements: Students who are eligible for or enrolled
under Medicare are not eligible to enroll under this policy nor
are students on a "Qualified Tuition Reduction Program"
taking less than 7 credit hours (6 credit hours for Graduate Assistants).
However, The University of Arizona requires that all International
Students be enrolled in the Student Health Insurance, unless
one qualifies for an exemption.
Undergraduates
must be taking at least 7 units, or have a consortium agreement
with The University of Arizona to take some courses at a qualified
community college, with an overall total of 7 units (documentation
is required) or be in a co-op program. Seniors with less than 7
units (one time exception) who are completing their final graduation
requirements this semester (documentation is required) and who had
subscribed to the student health insurance in the Fall
semester are also eligible.
Graduates
must be in a degree or certificate program taking at least 3 units or 1 dissertation/thesis unit.
Graduate
Non-Degree must have applied to a degree program (documentation
is required) and be taking at least 6 transferable units, be in
a certificate program, or be a full-time student taking at least
9 units.
Note:
Once enrolled, you and your dependents, if enrolled under your
policy, will be automatically re-enrolled and billed the appropriate premium through The
University of Arizona Bursar's Office in future semesters (each
Fall and Spring) upon class registration. The Campus Health Insurance Office will notify students who are not meeting eligibility requirements through their official University of Arizona email address (@email.arizona.edu). If you wish to cancel your coverage, you must do so during an "Open Enrollment".
Enrollment/Disenrollment
Instructions: Access Student Link (available Monday –
Saturday 7am-8pm), select “Registration”, click on “Insurance”
then hit “Modify”. Make your selection from the menu
and click update. You will receive a confirmation message that your
selection was successful. The open enrollment period begins upon
class registration and ends the 14th day after the official start
of classes. If you have a problem, please contact the Campus Health
Insurance Office.
If
Student Link is not available, you may complete an authorization
form or stop by our office for assistance.
Last
day of the Spring Open Enrollment: January 30, 2008 –
The Student Health Insurance can be added, canceled or changed only
during an "Open Enrollment" period, unless
one qualifies for a pro-rate. There are certain qualifying events
that will allow one to come on to the plan outside of an enrollment
period providing arrangements are made within 30 days of the event.

Dependent
Coverage’s: An application
is required on all dependents that are new to the plan. This includes
students who had their own student policy and become a dependent
under a student policy. Please submit your completed application
to the Campus Health Insurance office before the closing of the
"Open Enrollment" period.
Communications:
All "Open Enrollment" notices and pertinent information
regarding the Student Health Insurance is sent to the student's
official University of Arizona email (@email.arizona.edu) address.
Insurance ID cards: Temporary ID cards are available at the Campus Health Insurance office upon request. Permanent ID cards are mailed to new subscribers directly from the Insurance carrier to the United States address you have designated in your Student Link. This mailing will include two cards, your insurance ID from Aetna and a card from Assist America.
