Post Doctoral
Fellows, Visiting Scholars and Visiting Professors are eligible
to purchase the Student Health Insurance. Enrollment is done directly
through the Campus Health Insurance Office.

Open Enrollment:
Post Doctoral
Fellows, Visiting Scholars and Visiting Professors are eligible
to come on to the plan during an open enrollment period or within
30 days of a qualifying event. A qualifying event means the date
of arrival to The University of Arizona, a new appointment or an
involuntary loss of coverage. If you have a qualifying event, please contact the Campus Health Insurance office at 520-621-5002 for premium amount and eligibility fee.

Eligibility
Fee: $200.00
New
Enrollees: Need to complete an application if coming onto the plan effective January 16, 2008 and provide documentation from the university department stating
your status, as well as, duration of stay or a copy of your DS/2019
form.
Dependents: An application is required for all dependent’s new to the
plan.
Payment
of Premium and Eligibility Fee: Premium and eligibility
fees need to be paid in separate amounts. Acceptable forms of payment
can be a personal check, cashiers check, or a money order made payable to UofA Campus Health. If payment of premium and eligibility fee is done through the department, the eligibility fee can be paid through an IBF, but the insurance premium would need to be paid through a simple journal entry. A simple journal entry form can be printed out from the university website by going to www.fso.arizona.edu. Select e-forms then the letter "J". For qualifying events, please contact the
Campus Health Insurance office at 520-621-5002 for premium amount and eligibility
fee.
