Arizona Board of Regents Student Health Plan

The student health
insurance can be added, canceled or changed only during
an enrollment period by either going through:
1)
Student
Link (available Monday-Saturday 7am - 8pm) select registration,
insurance, modify, make selection then click update or
2) Print out an authorization
form (pdf file opens in new window)
and either fax or mail it to the Campus Health Insurance Office
or
3) Stop by the Campus Health Insurance Office for assistance
- Enrollment
opens upon class registration and ends the 14th day
after the official start of classes.
- Check
Student
Link for accuracy of your insurance entry or cancellation
by selecting the "Financial" icon to view account.
- When
you order your coverage, you are ordering a specific *amount of
time.
- You
must reorder your coverage on a semester basis as it is
not automatically renewed.
- All
communications, including reminder notices are done through the
"Broadcast Email System" to your official University of Arizona
email address @email.arizona.edu, see "Official
Email" policy.
- Make
sure you meet the **eligibility requirements before you order
your coverage.
- Students
applying for dependent coverage, who are not currently
enrolled as a dependent under the student health insurance must
begin the enrollment process at the Campus Health Insurance Office
to complete necessary applications.
- If
you are new to the plan, be sure to stop by the Campus Health
Insurance Office to pick up a temporary insurance ID card.
- Visiting
Scholars, Visiting Professors and Post Doctoral Fellows must enroll
and pay the insurance premium and eligibility fee through the
Campus Health Insurance Office.
- Graduate
Assistants/Associates have health insurance benefits paid for
by the University of Arizona providing they are officially hired
and have signed and filed with The University of Arizona a "Notice
of Appointment" prior to employment and taking a minimum of 6
units of graduate credit. Once the student health insurance is
ordered the Graduate College will credit the student Bursar’s
account the full premium for "Student Only" coverage. Graduate
Assistants/Associates must order their coverage
each semester, as this is not automatic.
- The
student health insurance is mandatory for all international students,
regardless of classification or number of units, unless one qualifies
for an exemption.
Once the
enrollment period ends there are no exceptions to adding,
changing or canceling your policy, unless you qualify for a pro-rate.
There are certain qualifying events that will allow a student to come
into the plan outside of an enrollment period providing arrangements
are made within 31 days of the qualifying event. To see if you qualify
for a pro-rate, please either call, email or stop by the Campus Health
Insurance Office.

- Last

- Eligibility
requirements: ** Students who are eligible for or enrolled
under Medicare are not eligible to enroll under
this policy nor are students who have qualified tuition fee waivers
taking less than 7 credit hours (6 credit hours for Graduate Assistants).
Undergraduates must be taking at least 7 units, or have
a consortium agreement with the University of Arizona to take
some courses at Pima College or Cochise Campus, with an overall
total of 7 units or be in a co-op program. Seniors with less than
7 units (one time exception) who are completing their final graduation
requirements this semester (documentation is required) and who
had subscribed to the student health insurance last Spring/Summer
semester are also eligible. Graduates must be in a degree
or certificate program. Graduate Non-Degree must have applied
to a degree program (documentation is required) and be taking
at least 6 transferable units, be in a certificate program, or
be a full-time student taking at least 9 units.

page or by requesting a "Benefits
Summary" from the Campus Health Insurance Office.
- Payment: Premium
payment should be made directly to the Bursar's Office and made
payable to the University of Arizona or through Student
Link by selecting the "Financial" icon. An installment

is available to students, providing arrangements are made through
the Campus Health Insurance Office during the enrollment period.
