GRADUATE
ASSISTANTS/ASSOCIATES have health benefits paid for by The University
of Arizona, providing they are officially hired and have signed
and filed with The University of Arizona a "Notice of Appointment"
prior to employment and taking a minimum of 6 units of graduate
credit. Once the hiring documents have been processed and you show
enrolled for your insurance coverage through Student Link,
the Graduate College will credit your student Bursar's account the
full premium for "Student Only" coverage. You may view the Graduate
College credit through your Student Link by selecting "Financial".
These credits are applied after the close of an enrollment period and when
applied the data line will read "GA Coverage".
IMPORTANT NOTE : Upon class registration, you and your dependents if enrolled under your policy will be automatically re-enrolled and billed the appropriate premium through The University of Arizona Bursar’s Office in future semesters (each fall and spring) whether or not your Graduate Assistantship continues. The Campus Health Insurance Office will notify students who are not meeting eligibility requirements through their official University of Arizona email address (@email.arizona.edu). If you wish to cancel coverage you must do so during the published “Open Enrollment”.
Enrollment/Disenrollment
Instructions: Access Student Link (available Monday
– Saturday 7am-8pm), select “Registration”, click
on “Insurance” then hit “Modify”. Make your
selection from the menu and click update. You will receive a confirmation
message that your selection was successful. The open enrollment
period begins upon class registration and ends the 14th day after the official start of classes. If you have a problem, please contact the Campus health
Insurance office.
If
Student Link is not available, you may complete an authorization
form or stop by our office for assistance.
Last
day of the Spring Open Enrollment: January 27, 2010 –The Student Health Insurance can be added, canceled or changed only during an "Open Enrollment" period, unless one qualifies for a pro-rate. There are certain qualifying events that will allow one to come onto the plan outside of an open enrollment period providing arrangements are made within 30 days of the qualifying event.

Dependent
Coverage’s: An application
is required on all dependents that are new to the plan. This includes
students who had their own student policy and become a dependent
under a student policy. Please submit your completed application
to the Campus Health Insurance office before the closing of the
"Open Enrollment" period.

Communications: In compliance with The University of Arizona email policy, all "Open Enrollment" notices and pertinent information
regarding the Student Health Insurance is sent to the student's
official University of Arizona email (@email.arizona.edu) address.
Insurance ID cards: Temporary ID cards are available at the Campus Health Insurance office upon request. Permanent ID cards are mailed to new subscribers directly from the Insurance carrier to the United States address you have designated in your Student Link. This mailing will include two cards, your insurance ID from Aetna and a card from On-Call International.
