Fall 2007

GRADUATE ASSISTANTS/ASSOCIATES have health benefits paid for by The University of Arizona, providing they are officially hired and have signed and filed with The University of Arizona a "Notice of Appointment" prior to employment and taking a minimum of 6 units of graduate credit. Once the hiring documents have been processed and you have been enrolled for your insurance coverage through Student Link, the Graduate College will credit your student Bursar's account the full premium for "Student Only" coverage. You may view the Graduate College credit through your Student Link by selecting "Financial". These credits are applied throughout an enrollment period and when applied the data line will read GA Coverage.

Note: Once you enroll, you and your dependents, if enrolled under your policy, will be automatically re-enrolled and billed through The University of Arizona Bursar's Office in future semesters (each Fall and Spring) upon class registration unless you cancel your coverage during an "Open Enrollment" period. Students not meeting eligibility requirements will be notified by email once the Campus Health Insurance office begins running eligibility reports.

Enrollment/Disenrollment Instructions: Access your Student Link (available Monday – Saturday 7am-8pm), select “Registration”, click on “Insurance” then hit “Modify”. Make your selection from the menu and click update. You will receive a confirmation message that your selection was successful. You may begin enrolling/disenrolling for your coverage upon class registration throughout the open enrollment period. If you have a problem, please contact the Campus health Insurance office.

If Student Link is not available, you may complete an authorization form or stop by our office for assistance.

Last day of the Fall Open Enrollment: September 4, 2007 – This is the last day to add, cancel or change your coverage, unless you qualify for a pro-rate.

Coverage period: August 16, 2007 through January 15, 2008

Dependent Coverage’s: An application is required on all dependents that are new to the plan. This includes students who had their own student policy and become a dependent under a student policy. Please submit your completed application to the Campus Health Insurance office before the closing of the "Open Enrollment" period.

Permanent Insurance ID cards: Cards are mailed out to new subscribers directly from the Insurance carrier to your United States address designated in Student Link.

Communications: All "Open Enrollment" notices and pertinent information regarding the Student Health Insurance is sent to the student's official University of Arizona email (@email.arizona.edu) address.
