Exemptions for International Students

Only international students need to contact our office for exemption requests.  If you qualify for an exemption, you MUST notify the Campus Health Insurance Office of your request during an "Open Enrollment" period and each semester thereafter.  Failure to do so can result in being liable for paying the insurance premium.  Please note that the cancellation of the Student Health Insurance is not permitted through Student Link.

If you register for classes in any of the following semesters, your exemption request must be received by the deadline date:

Summer 2008 Pre-session and Summer 2008 session I coverage period – Deadline June 23, 2008

Summer 2008 session II coverage period – Deadline July 28, 2008

Fall 2008 coverage period – Deadline September 8, 2008

Winter 2008 and Spring 2009 coverage period – Deadline January 28, 2009

In most cases the following circumstances are acceptable exemptions from enrollment in the Student Health Insurance:

When submitting your exemption request and required documentation to our office, please include your name, student ID number and the semester in which are requesting the exemption. Once received, you will be notified through your official university email address (@email.arizona.edu) within five business days to let you know if we can approve you for an exemption from the Student Health Insurance or not.

1. If you have a government sponsor who provides insurance for you that meets or exceeds the Student Health Insurance Plan – Please fax, email or bring to our office the name of your sponsor. 

2.  If you are a J1 visa holder in a program provided by the Department of State (DOS) through a sponsor or in a USAID program-please fax, email or bring to our office the name of your sponsor/program.  

3. If you (or your spouse or parent) have a US employer who provides health insurance for you - Please have the employer’s Human Resources Department complete the “Exemption Request” form.  When completed, please either fax, email or bring to our office.

4. If you are a UA Faculty/Staff Member or a dependent enrolled in the University sponsored health plan – Please fax, email or bring documentation of your enrollment in the health insurance plan to our office.

5. If you are doing your studies outside the U.S. for a whole semester – Please fax, email or bring your request and documentation from your College verifying your circumstances to our office.

6.  If you are registered for only a Summer pre-session or Winter session course – Please fax, email or notify our office of your exemption request.

7.  If you are a CESL student - Please fax, email or bring to our office notification of your status
