I completed a Risk Assessment Questionnaire (RAQ) in the past. Do
I have to do another one?
If you have
changed your position, animal(s) you are working with, types of
exposures, etc., then we need a new RAQ to re-evaluate your situation.
If an extended period of time has passed since your last RAQ was
completed, we may ask for a new one to insure that our records are
still accurate. Additionally, the Animal Hazards Program Committee
occasionally revises the RAQ and if content changes substantially,
you may be asked to complete a new one.

What is the appointment at Occupational Health for?
The appointment
with a provider at Occupational Health is an information exchange.
The provider will review your Risk Assessment Questionnaire with
you, discuss any health issues or concerns you may have, and ask
for additional information where needed. The provider will also
make recommendations for preventative measures you may want to consider,
including immunizations, titers, respirator use, etc. Immunizations,
skin tests, x-rays and titers can be taken care of here at Campus
Health as part of your visit.

Do I need to bring anything with me, like immunization records?
If you have
immunization records available, and they have not previously been
documented at Campus Health, it is a good idea to bring them and
copies will be placed in your record. Of particular interest are
tetanus, rabies, Hep A and Hep B immunizations, titers and boosters,
also any records on tuberculosis testing, including PPDs (Tb skin
test) and chest x-rays. These may or may not be part of the protocol
for the animal(s) you are working with, but it’s always good
information for the provider to have on hand in evaluating your
particular situation. If you do not have the immunization records
and are not able to include accurate dates for immunizations on
the Risk Assessment Questionnaire, the provider will discuss the
alternatives for handling this with you. There is nothing else you
need to bring with you.

Do I have to have any immunization that I don’t want?
No.
However, some positions require certain immunizations or tests as
part of the essential job function, or to protect the animals involved.
5.
Do I have to pay for any charges related to my Animal Hazards visits?
No. The Animal
Hazards Program covers the cost of compliance with program recommendations
and requirements.
