Health Insurance Plans
We
accept:

The
Campus Health Service is a participating provider to most
of the Blue Cross/Blue Shield, United Healthcare, Cigna and
Aetna plans. If you are a subscriber to any of these commercial
plans and would like to see if we are a participant under
your plan, please do the following.
Please note, not all of our services are contracted with these health insurance carriers.
- Call
your Member services
- Provide
them with the Campus Health Service Tax ID#: 74-2652689
- Give the provider name: Dr. Michael Stilson or Dr. Tejal Parikh
If
we are a participant under your plan:
- Present
Insurance ID card at time of service
- Additional Policyholder's Information required
- Your
co-payment, deductible and/or co-insurance responsibilities
would apply at the Campus Health Service
- Claims
are automatically filed for services rendered
If
we are not a participant under your plan:
- Campus
Health Service does not file with non-contracted insurance
carriers
- Request
an itemized Medical statement to submit to your insurance
carrier upon check out or download statements online

on Appointments/Patient log-in
- Select
“Click to Access Patient Link”
- Enter
your netid and password
- Type
in date of birth
- Select
“Account Summary"
- Itemized
Pharmacy statement: Must come in to request or if out-of-state

Charges
not paid on the day services are rendered will be
forwarded to The University of Arizona Student Bursar's account.
These charges should be paid as soon as possible to prevent
registration holds, cancellation of classes or other inconveniences.
You may call The University of Arizona Bursar's Office at
(520) 621-3232 for payment due dates. Campus Health accepts
cash, check, Visa, Mastercard or American Express.
For
the limited services available to UA Faculty/Staff, payment
for services rendered will be required at the time of service.

- Do
your benefits extend to Arizona and The University of Arizona
Campus Health Service
- Does
your policy have out-of-network benefits
- Does
your policy have an in-network deductible that applies to
office visits
- Do
you have policy year limits on services that may be available
at the Campus Health Service

you find that you have limited benefits, a high deductible
or no coverage except for emergencies, you might consider

Supplement
This
option is limited to the Campus Health Service and may compliment
your commercial health plan.
Student
Health Insurance Plan
This is a major medical insurance policy that provides coverage
nationwide and has a worldwide emergency benefit.
